Documents that may be required

You can submit any required documents by uploading a digitized version of the document directly to your online file (instead of mailing it in). This makes processing faster.

If you do send any required documents by mail, be sure to keep the originals as we may request them at a later date. If we request originals, they will be returned to you.

Following are examples of documents that may be required to establish eligibility for benefits:

  • Record of Employment Lien ouvrant dans une fenêtre.. A Record of Employment is an official document produced by your employer. It enables us to establish the amount of your benefits. Even if you do not have your Record(s) of Employment on hand, you can still apply for benefits, because your eligibility is established as of the date you file your application;

If you had only one employer and your gross weekly earnings were the same for the 26 weeks prior to the date you would like to start receiving benefits, you can ask to receive provisional benefits. When we receive your record or records of employment, they will carry out a new analysis of your application and, if applicable, adjust the amount of your benefits.

  • written confirmation from a doctor confirming the number of weeks of pregnancy and when the pregnancy was interrupted
  • proof of your immigrant status (e.g. forms IMM-1000 and IMM-5292)
  • a power of attorney to act on behalf of a person who is incapable of managing his or her affairs and a medical certificate confirming the person’s incapacity
  • the Revenu Québec and Canada Revenue Agency notices of assessment
  • large-sized birth certificate
  • a copy of the application for a placement order filed with the court for the purposes of adoption. The placement order indicates the date the order was filed at the court and the amount of the court office fees.
  • a copy of the letter you received from the centre jeunesse (youth centre) for the purposes of the Québec Parental Insurance Plan. The subject of the letter is confirmation of the information in the adoption file.
  • the letter (attestation) from the Québec Secrétariat à l’adoption internationale (SAI), prepared for the purposes of the Québec Parental Insurance Plan (form 1 or 2)
  • the death certificate of one of the parents
